Seasonal Worker Requirements

We, at St. Rose CYO, strive to create a safe environment for all our parish and community members.   As directed by the 'Diocese of Rockville Centre - Office for the Protection of Children and Young People,' all St. Rose CYO seasonal workers must comply with the U.S. parish and diocese safety practices and guidelines. 

All St. Rose CYO seasonal workers must complete the following requirements to become active workers.

  1. Click here for the Seasonal Worker form.  
  2. Click here for the Request for Taxpayer Identification Number and Certification (W9) form.  Mail page-1 of form to St. Rose of Lima CYO at 2 Bayview Ave, Massapequa, NY. 11758.  Signature and SSN are required for form to be valid.
  3. If you are a seasonal worker (age 18 and over), click here for the Diocese of Rockville Center's Protection of Children & Young People webpage and complete the three (3) safe environment requirements.  See below for supporting documentation;
  4. If you are a seasonal worker (age 17 and under), click here to read and electronically accept the St. Rose CYO Code of Conduct (U18).
